The Golden State Tour (the "Tour")

Attorney David Song, from Anaheim, CA, is licensed and admitted to practice law in the state of California. The Golden State Tour ("TOUR") was started in 2024. The ultimate goal is to provide free legal services to senior citizens residing in California who does not have access or the resources. Below a list of legal documents/services available on the Tour at no cost to client. Other services not listed are available at a reduced fee.

Power of Attorney

A power of attorney is a legal document that allows someone else to act on your behalf.

Advance Health Care Directive

You have the right to give instructions about your own health care. You also have the right to name someone else to make health care decisions for you. This form lets you do either or both of these things. It also lets you express specific wishes regarding other matters such as donation of organs, designation of your primary physician, etc.

Last Will and Testament

A Last Will and Testament is one legal document that controls how your property and obligations are distributed upon your death. A valid Will allows you to leave assets to people and/or entities according to your wishes.

Final Disposition Instructions

These instructions give you the opportunity to specify how you wish to have your remains dealt with (i.e., cremation or burial); to provide details of any prior arrangements, and to designate the person(s) to carry out your wishes.

HIPAA Authorization and Waiver

The HIPAA Authorization and Waiver authorizes your health care providers to release information concerning your otherwise confidential medical information to the individuals you have designated to act on your behalf in the event of disability and to any other individuals whom you would also want to have such access.
